I carry a small panel of patients so that I can best deliver comprehensive and personalized care. I specialize in treating complex cases of bipolar, psychosis, depression, anxiety, addiction issues, dual-diagnosis such as ADHD and depression combined with substance use, and behavioral issues. Copays, deductibles and coinsurance are due at the time of service. Emergency/Crisis Care We do NOT provide Emergency/Crisis Care - Please Call 911 or call/Text 988 Suicide and Crisis Lifeline or Seek care/help at the nearest Hospital/Emergency Department, and follow your Emergency/Crisis Plan. No Refund Policy. We have a strict no refund policy. Our providers make themself available for your appointment and their time is blocked for you. To ensure they are compensated for their time, it's important that we have this policy in place. Payment Policy: Payment is due at the time of booking the appointment, if no payment is made within 72 hrs of booking, the appointment will be cancelled. If you are using insurance to pay for your appointment, please ensure that you have provided authorization to bill your insurance and provided a payment method for co-pays/deductibles. Rescheduling Policy: You can reschedule your appointment 72hr prior to the appointment time by contacting us at 248-983-1070 or mjoz@eztelepsych.com, your payment will be transferred to the new appointment, this can only be done one time. No Show and Cancellation 48hr before the appointment Policy - you will be automatically charged a $75.00 fee. Disability-FMLA Paper-Work/School, or any Legal Case Evaluation Policy: These services are only available to our established patient(s) as we know them, know their history, know their Mental Health condition(s), treatment plans and we can confidently complete these types of paperwork. An establish patient is one that has been a patient with our practice for more than 6 months, or has had at least 4 or more appointments.
